3a) Front door emergency exit was locked with dead bolt and did not automatically unlock when the knob or handle was turned and the back sliding glass door emergency exit was locked preventing the door from opening. 4b) Licensed front room did not have a second emergency (during visit Provider asked to take this room off license and asked to license living room and dining room instead).
